I had a phone only contact with a mom tonight. I can't see her until
Thursday. late PM. I am trying to find an alternate lactation
consultant for her but meanwhile... I have a puzzle.

History
Csection / difficult latch in hospital - lots of screaming - home day 3
at home still difficult latch but better for 2 - 3 days
saw another LC, set up with finger feeding (seemed poorly taught)
    and "finger exercises" ?suck training?
    and cup feeding
tryed supplementing with feeding tube device at breast but really
    agitated baby more
did best with cup feeding, screamed a lot with finger feeding
weight check at 9 days good
mom noticed tight frenulum, clipped by doctor at her HMO at 9 days.
Her description of preclipped frenulum included upper part of a
heart shaped tongue when crying so it sounds like that was a good
    call on mom's part.
day 11 finger fed first then latched well for 15" all feedings
day 12 (Today) baby refused all latches, worse than ever, very agitated
with all attempts to get to breast. Likes finger feeding.

Other LC says she can't think of anything else to try. Mom's
"technique if fine", "breasts and nipples are normal" and to try
calling Carla, "She is really good with cases like this". Well - Carla
smiled, frowned, cryed - considered own feelings, am I complimented,
dumped on or challenged? I don't know, but I said I'm sure we have
other tools and avenues to explore. Mom was in tears by this point,
really doesn't want to give up. Seems willing to try someone/something
new.

I asked mom to check frenulum clipping site. She says it looks like a
"cankor sore - whitish". I'm wondering if it might be infected. What
does a healing clipped frenulum look like? I admit, I've never seen
one. She will take baby in to clipping doctor tomorrow for a site
check.

I told her we might consider the rebirthing bath, more questions on
that to follow in another message (trying to keep subject lines well
defined).

I told her we might try supplementing at the breast again (this
technique frequently works well for me with a baby needing quick flow).

I told her we might as a last resort try nipple shields (with requisite
warnings, precautions and weaning plans).

Meanwhile I told her to stop the "finger exercises", it was making the
baby scream and stick to "happy" mouth contacts (thank you to the list
member who originally used that phrasing - I really like it). Continue
finger feeding after describing the technique more over the phone and
continue pumping. I probably can't see her until day after tomorrow.

First and BIG question - just what does a healing vs an infected
clipped frenulum look like?

Second question - any other suggestions, tools, directions I should
consider?

Thanks in advance, Carla (just north of Washington DC where we have
MANY young in the profession LCs setting up unmentored practices).
