I would love to be corrected if I'm wrong, but if the LC is part of the
hospital  "marketing" package of service and  the HOSPITAL does not have a
disclaimer under its  service listing that it may be limited by the choice
of each physician, then when physicians sign on with a hospital aren't
they accepting this hospital provided  maternity service ? Especially in
the cases where it is "free"?

No service is free.  Some services are simply not billed separately - they
are rolled into a single daily charge.

Physicians do have to observe certain basic rules and restrictions (must have
a license and malpractice insurance, credentials are in order, etc, as well
as we may not always get the brand of gloves or equipment we prefer, if we
cannot make a good case to administration).  However, doctors do not have to
accept all a hospital provides.

Hospitals market the AVAILABILITY of services.  It is up to the doctor and
the patient to choose what they wish to take advantage of.  Just because the
hospital provides 24 hr epidurals does not mean everyone must get one.
