I have seen 2 cases of a baby's strong suction (compensating for a
sub-optimal latch, low supply due to pregnancy or other) causing small but
painful "balls" which were like a raised fluid- filled cyst on the nipple
tip. A couple of days of improved breastfeeding and they just went down and
the pain subsided. No rupturing was necessary-skin remained intact so
healing was fast. There was no evidence of white spot, just this fluid
filled  "balloon" that was difficult to see straight on, but when looking at
the side view of the nipple against a plain background became evident.
