I think it is important with C-sec moms for baby to be immediately skin-to-skin with mom with ample opportunity to mouth the breast/nipple before anyone else touches baby's mouth.  These babies are at special risk of being colonized with pathogenic hospital bacteria because they are not colonized in mom's birth canal.

I agree that probiotics are a good idea for baby.   The babies could be infected and may also need some treatment.  There are many different types of probiotics and some of them are more appropriate for use in different situations (I do not have the knowledge of which are appropriate for which situations) so perhaps a more targeted probiotic would be helpful.  I would also culture mom and baby to see if the antibiotics are appropriate for her infection.

For any mom with nipple damage, I ask them to rinse after every feed with plain or saline water, wash nipples once a day with soap and water and use virgin coconut oil after every feed.  It is protective against staph and candida.

I know moms who have had some success with taking raw garlic for long term breast pain.  2-4 cloves a day crushed, cut into pieces and taken like pills or added to cold tomato sauce or spread on toast or cold salads.
