I do not wear gloves when assisting healthy mothers and babies to breastfeed.  However, I think that it would be prudent to wear gloves when assisting a mother and an NICU baby.  The reason for wearing the gloves would be to protect the fragile infant from the LC's hands, and also to help prevent the spread of nosocomial infection.  I would explain the reason for wearing the gloves to the mother.
