I participated in what I suspect was the Ross Labs survey after Alyssa's
birth.  A questionaire came in the mail, twice or three times, I beleive.
 I had her at home, but did pre-register at the hospital in case....  The
questions were something like (disclaimer: this was 7 yrs ago):
for each of the following months did you breastfeed your baby?
for each of the following months, did you give your baby any of the
following milks (all name brands of abm, and I think, straight cow milk
too)
for each of the following months, did your pediatrician talk to you
about or give you any of the following (all name brands of formula).
they might have asked about solids, also.
        While I figured it was the Ross labs survey, I answered it, cause
I wanted them to count at least one woman whose ped did not give her a
drop of abm (or even mention it) and who bf exclusively...
