I just use a memo sized notebook (the fat, plastic covered durable sort)
and log all calls and messages with a code for whether I took the call
directly or it was a voicemail.  I check off once I've called back, and
put a little m on the check if I've just left her a message.  If it is a
client I've already seen, I make a note in my computerized client
database of the date and reason for the call and my recommendations.  If
I make the call myself, I just log it in my database.

Computerizing the system would probably be the best thing for a group
practice.  You can use a database program, and write a little data entry
page with your info.  THen you can set the database to print you a daily
report of all the calls taken.  You can even set it up to calculate the
time spent on each call if you put in the start and end time of the
call.  I'm still using Lotus Approach, but it's no longer being updated
regularly, so eventually it's not going to work with the new Windows
versions.
