We do not have a nipple shield policy - I was going to implement one but because of the resistance have let it ride a little longer. However I do have all nipple shields autoclaved and in my room. With the shields is a book. The staff write the womans name into the book and sign for its removal from my office. This way I can follow-up the womans breastfeeding progress and also keep an eye on the staff who give them out! Stapled to each package is a handout I have put together myself. This sheet explains to the mother the appropriate use of a nipple shield and the risks involved. - I am happy to share this with you.
I have found the use of nipple shields has dropped dramatically - in fact  its only me that is signing the book maybe once a month or so now! - I have to feel comfortable with this process. It allows staff to feel independant as practitioners but they have to think about it!
