Dear Jan,
Dr. E. Jayne brought this situation up at a conference she spoke at.  Her
recommendation was to use a nipple shield until the baby can accommodate
mom's nipple.  This will mean the nipple does get a bit squished into the
shield but this does not harm the nipple nor compromise getting the baby deep
onto the breast.
I had a mother recently that I used the shield on one of her breast because
the nipples were not equal, one was much longer than the other.  The baby did
fine going back and forth from shield and bare breast and the mother was
actually more comfortable nursing on the side with the shield than without.
