I generally don't recommend that mother's with breast reductions start
supplementing so early.  We watch the baby for energy, hydration,
jaundice, and diaper output, and generally they do okay for 3 days on
the colostrum the mom is able to produce.

It's never too early to start using a Lact-aid, if the baby really needs
the supplement.

Several of the BFAR moms I've seen have used a combination of
domperidone and goat's rue as galactogogues, with good results.

Milk supply seems to be very, very variable in BFAR mothers.  I've seen
one who had free nipple grafts make all but 4 oz a day of baby's needs
(after a LOT of work with the Lact-aid and galactogogues), and others
who had pedicle technique surgeries that are supposed to be less
destructive) make only drops despite feeding and pumping frequently.
The most important things to teach mom (beyond the important basicis of
positioning and latch)  are how to watch the baby for signs of continued
hunger, and how to calibrate supplementation.
