Aterax is an alternative antihistamine, that I think works well. Can
you ask about that?

I strongly believe that the first step is to get rid of the notion that
these are in any way "gifts"! They are advertising samples, nothing else.
By promoting the concept of a gift, patients are led to believe they are
somehow their "right". This is perpetuated by getting coupons in the hands
of expectant moms so they bring them to the hospital to ask for their gift
and are then a little miffed if they don't get it. (A technique that grows
as the number of hospitals opting out on playing the game increases.) After
all, the coupon says it's their right to get it (and don't we all like to
get something we perceive as free?) Nurses are put in the role as the
givers or refusers of these so called gifts and who wouldn't rather be
perceived as a giver rather than one who refuses to give a gift?

Subject: Zyrtec and low milk supply

I met with a client last night who has a low milk supply and a nine day old
who has not stooled since Wednesday. He is nine ounces below birth weight and
was being supplemented until Wednesday (due to "high bilirubin) when the
pediatrician said it was OK to quit the supp. She had not been pumping during
that
time and he was getting 8 oz. per day! When we pumped she got 20 ccs after
twenty minutes of pumping with a Classic(single kit, her choice, switching every
5
minutes). She also c/o being thirsty all the time. He is not transferring
much at the breast either so I recommended that we get back to supplementing
while working on increasing the milk supply. Mom is also very anxious and hyper.

She is taking Zyrtec and using Flonase for "allergies and has been for the last
two years. She said the Zyrtec "helps her sleep."
Has anyone else had this experience with Zyrtec?

Subject: Marijuana

Hello Lactnetters! Marijuana has reared it's ugly head, again. I am
interested in knowing how other NICUs handle the marijuana issue. And any new
or old research that someone may have, please send it my way. I live in a state
that allows marijuana for medicinal use. Does anyone have experience or
knowlege of how this impacts it's use and breastfeeding? You may reply to me
personally.
Thanks,
Suzanne Bowes RN, IBCLC

Subject: HMF in mother' milk, preparation under laminar hood?

I am searching for information about safety concerns/recommendations about
preparing human milk with the additive of human milk fortifier in a
NICU/SCN setting.

To prepare all the infant's feeds in our SCN, currently our hospital uses
the clean counter beside the main breastmilk refrigerator and adjoining
sink .

Is a laminar flow hood now being recommended? Our nurse manager is updating
our SCN policies and stated she thought JCAHO's recommendations were for
laminar flow only.

This is the information I've found on line so far....

http://www.adhb.govt.nz/newborn/Guidelines/Nutrition/HandlingFeeds.htm

These detailed 2003 "Guidelines for Preparation of Formula and Breastmilk
in the Health Care Setting" by the American Dietetic Association:
http://www.eatright.org/Public/NutritionInformation/92_17242.cfm

Canada's health professional advisory r/t powdered infant formula
preparations:
http://www.hc-sc.gc.ca/food-aliment/mh-dm/mhe-
dme/e_enterobacter_sakazakii.html

Only the Canada site mentions in passing preparation under a laminar flow
hood.

How many NICU/SCN's are using Laminar flow hoods to add HMF to human milk?
Why and why not? Any suggestions or additionsl references for me?

Thank-you in advance for the help,
Rachel A. Miller RN, BSN, IBCLC
