There are different expectations for babies with cardiac issues. First, 
I would let the baby (and mom) enjoy that 5 hours of sleep at night. 
During the day, mom can offer short, very very frequent breastfeeding. 
If baby needs more milk, mom can pump and feed some extra with very 
careful pacing, perhaps with a Haberman feeder on slow flow. 5 oz a week 
weight gain is actually good for a little girl with cardiac issues. We 
just need to watch baby to make sure that she does not decompensate and 
start to gain less. Mom can also do some extra pumping (pump for a few 
minutes after each bf) so that her milk supply is generous and the baby 
does not have to work as hard at breast.
An IBCLC or OT with good skills in assessing infant work and flow 
tolerance should be able to help more.
