Just a quick up-date on the twins I'm working with.  To re-cap, they were
born at reasonable weights (both over 6 lbs) and close to term, however were
almost 4 weeks old and on bottles when I first saw them.  We got them to
breast with nipple shields, nursing sequentially (mom unable to coordinate
simultaneous feeds, and babies too difficult to latch).  They've been on
shields for 2 weeks and weigh 8.7 and 9.8 respectively.  The diaper rash on
little girl, and mom's sore nipples cleared when we added bactroban (topical
anti-biotic) to the anti-fungal tx mom was using.  Larger pump flange
resolved the rest of her nipple pain.  I went back and spent a couple of
hours (in bed :) with them yest.  We got both babies latched without
shields.  Little girl took in 3.8 oz. and little boy slightly less.  Boy
were they happy!

Had another neat contact by ph. with mom of baby who had been 4 mo in NICU
and transisitioned to breast with shields.  Baby is 15 mo old (12 months
corrected gest age) and still nursing.

So yesterday was a good day, to think that these more at-risk kids are all
going to have the best start. Aren't we lucky to get to do this work?!!!
