Hi,
I am hoping someone can help me with this mom I've been working with for 8 months. She is a very large-breasted mommy, and had to use a nipple shield for the first 6-8 weeks postpartum to achieve a good latch. Her baby has latched and nursed well without the shield for 6 months now. Baby's birth weight was 6-12. 
This mom's baby has always  been incredibly slow to gain. At this point, the baby's weight is at 13-12. The pediatrician is now becoming concerned after several months of NOT being concerned. Mom's baby is eating about 6 ounces of solids daily, and actually will fall asleep during the morning feeding, right in the high chair. The mother takes baby to daycare, and they can only get the baby to take 2 ounces of breastmilk by bottle in the morning, and 2 ounces at night. Stools are runnier than what you would expect in an 8-month old, but the feedings are mostly breastmilk feedings. Mom has pre-and post-weighed her baby after a good breastfeeding in the morning, and the baby weighs about 5-8 ounces more than before the feeding.

Baby is also developmentally ok - sitting w/o suppport and mentally responsive. This baby is alert, happy, and always smiling. The parents are very attentive to the baby. 
The ped is now sending mom to a specialist to look at the possibility of metabolic disorders. Mother told me today that baby's dad has a father who has had Crohn's disease.
