To build on Fay's response.

The brand new 2011 HMBANA reference, Best practice for Expressing, Storing and Handing Human Milk, has a sample Agreement for Use of Donor Milk in the appendix.  www.hmbana.org
It also contains an expanded section on storage times, which is always a big question from healthcare professionals and mothers.

Although it does not have a specific education handout on donor milk (benefits, etc.) the layout of this reference (strategy / rationale / quality of evidence) makes the information readily available to draft a handout with the most current information.
