I'm feeling inspired by this great discussion.
Here are some possibilities for charts:
"Previous hopes to bf unfulfilled due to lack of: medical / hospital /
family / spousal support"
"Incomplete bf education prenatally; pedagogical support offered
successfully"
"Incomplete transfer of milk at this feeding" "Contact at breast did not
result in transfer today." "Needs support at next feeding." "Direct
support / education / assistance at following situation : latch not
optimal / clenching / etc."
"bf to this point unachieved"
"continuing attempts at bf"
"normal course of bf interrupted by: early intro of artificial (nipple,
substitutes)"
"baby not showing signs of normal newborn behaviour : not yet stooling

Possibilities for mothers (After the brilliant "How does it feel?"):
"Do you have concerns about weight gain / milk supply / the baby's

where you'd like to be. Where would you like to go from here?"
"It sounds like things haven't come together yet." "Maybe we can do some
fine-tuning." "What do you think happened last time, so that you weren't
