I could not agree more with Laurie.  "Developmental delays" can be for a variety of reasons and honestly, some of the underlying issues result in smaller children, period.  I worked for a while with heart infant and kids in the pediatric ICU.  To expect these kids to grow according to the growth curve is setting them for failure... They will be smaller, even if you increase calories in the feed.  I agree that consulting a pediatric specialist and a peds dietitian for an appropriate assessment would be my first step, rather than making decisions based on growth curve alone.  Overfeeding calories today may seem "appropriate", but the long term negative outcomes of overfeeding a child are far more concerning.
