Hi Marie,
A breastshell could help her heal the nipple. It would keep direct contact from clothing and pads away from the trauma site. One can place a rolled up pad or a pad cut into thirds, at the bottom of the shell to collect leaking milk. The treatment prescribed does not seem to fit the complaint. (aside from the fact that it also is not effective.)
the other issue is that you mentioned trauma of one nipple, so even if she cut back from that side to allow the lesion to heal, she could theoretically nurse from one side only (the unaffected side) without compromising the baby or the milk supply expressing milk on the affected side. After the wound has healed she resume feeding from both.
