Dear Theresa,
First of all, I agree with Virginia that the great majority of the time 
this a non-issue. All things balance out and breastmilk is beautifully 
designed to feed babies well.
If, on the other hand, you have a baby who is really gassy or struggling 
with weight gain, or a mother that is worried even if you reassure her, 
you can suggest that before the mother feeds have her give her breast a 
vigorous massage to loosen up the fats so the baby gets them right away. 
Someone suggested this to me years ago, Cathy W-G?, and called it a 
"milk shake"! Moms find this hilarious and it certainly won't hurt 
anything. It also makes mothers feel like they are doing something to help.
It makes sense that to "homogenization" the milk would help balance the 
fats and make them more accessible to the baby right away.
Just an idea.
