I have a mother with a 2-week old whose milk supply was delayed for 5 days, 
then the milk came in to the point of moderate engorgement. Mom went and had 
lymphatic drainage massage, which helped to eliminate her plugged ducts and 
engorgement, but has never had any luck getting her milk to let down. Her 
breasts feel full, but alas no milk flow. Even the massage therapist was 
perpexed as to why she did not see any milk flow during or after the 
massage.

We had the baby come in at 10 days of age, and she only takes 4ml at breast 
(with a test weight) - I see virtually no swallowing. Mom has been using a 
supplementer at breast for 2 weeks now.

We also had our local compunding pharmacy create an oxytocin spray - mom 
says she thinks it helps a little bit, but she is still only able to express 
8ml after using the spray.

What is going on here? I think that possibly this mom's ducts are not 
structured normally, and that they are not reaching her nipple pores - is 
this possible?

If anyone has ever seen this, I would sure be glad to hear from you. This 
mom has been the greatest rouper, but she started crying on the phone today, 
and I really don't know what else to do at this point.
