Hi Diane,
I regularly advise parents to hold the bottle so the nipple is only half
full of milk, keeping the milk just at the holes if the baby handles
milk flow well, and just below the holes if the baby handles flow more
poorly.  I have to promise them that all the air fears are marketing
hype to sell bottles, and the baby seems so much more comfortable.  I do
notice nice, big burps when the baby is held after the feeding.  (I also
discourage back smacking, gentle rocking in a sitting position brings up
air if the baby has any!)

My reasoning is to reduce overfeeding, and to make the feeding more
comfortable for babies who have respiratory or anatomical problems.  And
more active too, I want the baby to be in control.
