Hi Jennifer,

From the story you sent it sounds like the problem is a hind-milk, fore-milk imbalance.  There are several archives on Lactnet which discuss this and you will find similar stories of babies like this one.  Also the Breastfeeding Answer Book by LLLI has information on hind-milk, fore-milk imbalance on pp 25, 90, 123, and 281.  As you mentioned the mom has a heavy milk supply and the child chokes on feeds.  This is quite common with this imbalance.  It is also easy to remedy.  The mom needs to first pump out milk from the left breast by hand or pump expression.  Then feed on the left breast.  She should feed only on this breast if the baby is hungry again during a 3 hour period.  Do not give the right breast or pump the right breast during this time.  After approx. 3 hours give only the right breast with either pumping first or expressing slightly to remove the bulk of the foremilk and then nurse on right breast only.  Again if baby is fussy or needs to feed again feed only on the right breast during this time and do not pump on left breast.  After approx. 3 days of doing this the baby should have normal stools and will be sleeping better and less fussy.  If mom  notices this happening again with an occasional green stool she can remedy it easily by using only one breast per feeding.

If there is an underlying allergy or other medical problem this should at least help and the child will receive some comfortable feeds until a possible allergy is found.  Sounds like the baby's GI tract may have been compromised with the short time of formula and may be the reason the child has brochiolitis now.   The doctor was right on in the suggestion to continue nursing.  I'm very impressed.  I hope the baby and mom will find relief before it's too late to go back.
