Thanks to a wonderful team of lactation experts, the Canadian Institute of
Child Health has published the (revised) National Breastfeeding Guidelines -
1996.
For those Health Care Providers who chose to get this publication
there's some very appropriate advice, I especially appreciated seeing the
treatment of thrush put into perspective ie -- Nystatin topically on mom and
infant are usual first line treatments -- often don't work, Clotrimazole or
Gentian Violet may also be used. BUT -- "Ketoconazole..., and Fluconazole
are other anti-fungal agents which can be used for the most resistant cases".

Now perhaps physicians who are approached by knowledgeable
breastfeeding patients will feel more comfortable about prescribing these
medications if necessary, now that they have some "back-up". (and from the
"government" at that!)
