Don't you wish you'd heard from this mom the day her baby was born?
Then you could have talked about starting pumping within 6 hours of
birth, developing a great supply in the first 2 weeks, letting baby
mouth the breast in a prone position, and all the other things we know
help maintain a long term milk supply for pumping moms.

Perhaps her baby can still be encouraged to explore the breast if placed
prone on mom's chest in between feedings when he's not too hungry.  Skin
to skin can still work its magic.  Mom should check the valves on her
breastpump, and should make sure she has the best quality pump.
Frequent pumping for a few days can sometimes build a faltering milk
supply, see if the mom can "powerpump" every 45 mins or so (when she is
awake, we don't want her waking up every 45 mins at night) for 3 or 4
days.

There seem to be two different schools of thought on timing of cleft
surgery.  The traditional school says the later the better, as there is
more tissue to work with.  The newer school (Dr. McGee and Denk who
spoke at the ILCA conference in Acapulco ascribe to) says the earlier
the better, because then normal feeding is maintained, and healing
improves due to the very rapid growth rate in the first weeks
postpartum.  Time will tell which approach is best for each particular
situation.
