 Found an article in Nursing Matters www.nursingmatters.org.uk by Pamela Morrison, June 2009 on 'Inadequate weight gain/failure to thrive in the exclusively breastfed Infant'.  Very specific plan to follow and discussion of supplementation.  (CBI RL MSF-5898)  Probably what you are looking for.  

There are several older articles by Kittie Frantz and by Nancy Powers on slow weight gain.  Pat in SNJ
