I am not saying that i don't 'dx' or refer for treatment when needed. My concern is that i am seeing babies for the first time only after ties are released and baby/mom is still experiencing pain/wt gain/xxxxxx problems or the issue they were having before is now worse. This is happening more frequently with lip ties than tongue ties. I wished that prior to the surgery a visit to an IBCLC would be  required to determine if this was the true issue. Since everything a parent reads tends to state that breastfeeding is so 'normal' they don't understand how important a correct latch is and how much trouble could be caused from a poor latch. 
I'm starting to gather a list off all the places these mothers are going to in my local area for revisions and will write them a letter asking hopefully for some kind of working relationship with them so maybe they will encourage mothers to get a LC consult first. My services are free so money is not an issue. 
From the parents information it seems like the first day or two after revision seems better but around day 3-4 is when tbey start to see problems so making a post consult with the IBCLC on day 3 maybe of benefit as well. Wonder if this is the time period when baby is experiencing more discomfort?
