  Halle & ALL,  I would look @ the problem under 3 headings
  1. Did she have an abortion or was it an intra-uterine death?
      Whichever it was has she come to terms with it, maybe it might be discussed with her by you or should she be referred to another professional as if unresolved it may be inhibiting her lactation?
  2. Did the baby ever really feed correctly, did staff "just" see the baby go to the breast & assume all was well & move on to another client? 
  3. Action now. Good attachment throughout plus do breast compression whenever there is a pause during feed from the 1st breast, good attachment, lactation-aid/ supplementer on the 2nd breast. E.B.M.[expressed br.milk] +/- Artificial milk as supplement. After as many feeds as possible hand + double-pump express. I suggest [privately as some might be embarressed] that if the woman has a willing partner that he suckle her breasts for about 10 + 10 twice or three times daily immediately after she has finished expressing - this is very successful. She could continue with the herbal remedies, also what about Domperidone, see Jack's website plus Tom Hale's book Medications & mothers Milk.
