With the rate and prevalence of birth interventions, I no longer assume that
just because a baby is "at breast," the baby is actually getting milk.
Epidurals, Cesarean, forceps, vacuum extractors and/or induction &
augmentation of labor can ALL impair infants' sucking ability and
coordination. 

Some of you are familiar with my ABC Protocol: 
A (Access): get the baby in the restaurant. (mother is the restaurant)
B (Breastmilk transfer): make sure the baby is actually eating / getting
milk. 
C (Comfort): Keep the cook (mom) comfortable.
