Hi Jennifer,
Congratulations! It seems that you, mom, and baby have made great
progress. What I am picturing is that mom is lifting her large, heavy
breast up to the baby but needs to find a more natural position. I
think mom (with you) needs to experiment. What I am thinking is for
mom to be sort of laid back (semi-reclined) and maybe leaning to the
side a little (to the side she is to feed on), and her breast to be on
a pillow, and baby to be sort of in a cross cradle position and
probably needing support under him too. I am picturing his head to be
somewhat far out from mother (where her breast/nipple is) but his body
angled in towards her middle.  I don't use those breastfeeding pillows
that wrap around you, but would something like that help support moms
breasts? I have seen and heard of some mothers with large breasts to
use 2 surfaces, for example to sit next to the adult bed, to place her
breast on the bed which is where the baby is. I have seen some mothers
use a bedside table to place baby and breast on. It is important to be
very sensitive to mother's feelings because this can be embarrassing
to her, and she wonders how is she ever going to feed the baby in
public. Some mothers choose to express milk for public feeds, as it is
just not easy to do otherwise.
I once had a mother with large and pendulous breasts whose nipples
ended up down on her thighs. She was having a hard time getting
comfortable br. We had a session where I said just let your breasts
fall in a natural way and we experimented with the baby and found a
position that worked. I really don't remember what it was, but what
was not working  were the textbook positions that we usually suggest.
