I use the Monoject #412 12cc periodontal syringe (approx. 16mm diameter
inside barrel). I cut the barrel at the tip end to open it up, then take
out the plunger and put it in the other side, so that the "finished" side
is against mom's skin and the raw, cut side away from her. Mother pulls
slowly to suck the nipple out, and may hold it out for a few seconds.
This works well for inverted nipples, and would probably also work for
flat nipples, too (I haven't tried it with them). Best of all, it is a
very cheap solution, under $1!
