I have had a few Moms over the last 19 years with the same problem.  I 
keep a few of the glass flanges on hand for this reason and yes I teach 
them hand expression.  One Mom said that neither her mother or sister 
could BF because of the same problem.  When one of these women had her 
second baby she called asking to use the flange, rented a Classic, used 
hand expression  a few times a day (I find that about 5 minutes 3 to 4 
xperday really helps the supply) and was finally fully BF at 4 weeks.
