I am familiar with the Velcro dimpled nipple ring, which was made exclusively for the Lactation Institute but is no longer easily available. The goal of the ring was to hold the dimple open long enough for the area to dry before folding back together, but I have learned to work around it.  The pain of dimpled nipples is largely due to bacterial growth inside the dimple from the constant dampness. It may also be exacerbated if the area adheres and then is pulled apart when it is sucked out by nursing. 

I teach moms how to work the dimple open, wash it quickly, pat and/or blow dry, then apply a little polysporin (if it is sore) or lanolin (if prophylactically), then allow it to fold back in. We are usually able to manage the dimpled nipple in this manner.
