The _Core Curriculum for Lactation Consultant Practice_ chapter 17 is a
good place to go.  Starting p265 you'll find "Some published examples of
preterm infant feeding regimes are as follows" (5 examples and their
references).  In case your library has the journals but you can't lay hands
on the book here are the refs (w/o titles):
