Over-active letdown is often described as painful. Some women even say that the pain starts in thier backs and radiates down thier shoulders. Usually this intense feeling diminishes over time during the breastfeeding relationship. What for signs of oversupply because the 2 often go hand in hand.
Pain is a relative term. As long as the pain is only occuring during letdown and there are no obvious sign of other issues, I  would reassure mom that what she is feeling is normal and a good sign that she has ample amounts of milk for her baby!
