I wonder about whether this is, in fact, a normal finding, which I would think it was.

Was the dilation unilateral or bilateral? Did they ultrasound both sides?

I would discuss with mother how to generally manage oversupply and frequent plugs and mastitis.

(I think mom had to do a lot of pumping for a baby with cardiac surgery and we know this can mess

with a normal supply).

I also would discuss the possibility of releasing the posterior tongue-tie. I certainly understand a parent's reluctance

to do any further (even if minimal) on a baby who has been through enough already. But.....it could help the baby

drain the breasts more effectively and not clamp down etc.
