If you suspect tongue-tie but aren't sure, you can ask the mom how nursing 
feels to her. I had a tongue-tied son and nursing him felt like he was sawing 
back and forth on my nipple. External observation showed a "good latch" 
and "excellent positioning". As he has matured, I see a rolling action to his 
tongue when he swallows. Sam's dentist has also commented on this. (I wish 
he had been Sam's dentist seven years ago!) I think the only reason I 
maintained supply was that I was tandeming him with his then 3-year sister. I 
think that her ability to bring down the milk also helped him to gain 
adequately, however slowly. Since I had another breastfeeding experience to 
compare to, I knew something "wasn't right" with Sam from that first nursing. 
For a mom's first experience to be thus, it may be tough for her to find the 
vocabulary to tell you what she is feeling. Just "sore" can mean so many 
things! As an aside, we also had thrush for many months. Possible infection 
due to nipple damage? I hope this is helpful to you in figuring out further how 
to help this heroic and overwhelmed mom!
