Treating the tongue tie would be a start.
Donna Ramsay showed some interesting research on infant sucking at ILCA. 
Some babies use a lot of pressure when they suck, others used abnormal 
movements that distorted mom's nipple. The latch can look good and one 
of these things can be going on.
