I've been wondering about this, too.  Presently, we just have moms clean
right away with water, first cold, then hot.  However, I'm really not
comfortable with this.  What do you do if the piece falls on the floor?
I was much more comfortable when, long ago, pump kits were sterilized
after every use.  The biggest disadvantage was that the mother had
nothing to take home, but I could be confident the set was clean.

As far as putting dish soap in med cups, this is bad practice.  It
doesn't meet OSHA guidelines.  Anything in a med cup should be for oral
use and safe to swallow.  Anything left in a room must be appropriately
labeled.  What happens when the mother's toddler drinks the detergent?
