If mother can be convinced to use cold therapy, it can be applied continuously as long as a cloth is placed between the cold pack and the mother's skin. Personally, I used cold (refrigerated) or, as I remember, frozen or at least frosty, cabbage leaves. These were wrapped as a poultice around engorged (already small breasts and stretched by implants, since removed) breasts, continuously. And new leaves placed as the first ones became warm and wilty. Of course, used with expression as needed.
