Maybe ask the insurance company what they require? I've found that insurance companies are all over the map when it comes to their requirements for covering breast pumps.

I know that some Insurance company's are providing pumps for Moms, but some require a prescription for coverage, can anyone give me help with the verbiage in this letter.  I wrote the diagnosis for necessity in the baby, and that to ensure the production and extraction of this necessary milk, Mom needs a hospital quality pump. Does anyone have any further advise.  Thank you for your help.  (The note is signed by the Physician/provider that I work with)
