The following references may be of interest:

Kazi A, Kennedy KI, Visness CM, Khan T. 1995.  Effectiveness of LAM in 
Pakistan.  Fertility and Sterility 64: 717-23

Kristan A, Cooney MA.  1993.  Assessment of the nine-month lactational 
amenorrhoea method in Rwanda.  Institute for Reproductive Health: Paper 10, 
Georgetown University Medical Center.

If I remember correctly, the first one looked at the effectiveness of LAM 
for 12 months and the second one nine months.  I seem to recall that in 
Rwanda 419 women used LAM for 9 months and there were no pregnancies. 
Complementary foods were introduced at 6 months.
