Dr. Jen,
I am a fan of gavage feedings when the baby really needs the food but is
just not ready to eat (illness, tachypnea, prematurity). In the situation
you described (lethargic infant) I would keep the baby and mom skin to skin
as much as possible (or, if not possible, together as much as possible) and
gavage feed the baby. Allow him to show hunger cues, rooting, and proceed
from there. Of course keep the mom expressing.
