It is very common for women to not get pregnant for the first four months of TTC so I wound't necessarily say she is at "problem" stage yet. As women age, they have more and more anovulatory cycles (where they do not produce an egg) so between the normal time it take to get pregnant, the regulation of hormones after she starts cycling again and the chance that she is not ovulating every cycle could all be a part of why she has not concieved yet. My best advice to her would be to get a fertility awareness book (my favorite is Taking Charge of Your Fertility by Toni Weschler), read the book in full and start charting her cycles. This will help her find out if she is ovulating every month or at all, if she is having a long enough luteal phase to hold a pregnancy and if she is having intercourse on days most likely to result in pregnancy. I think charting should always be the first step in fertility treatments, and the fact that is starting right away is good, it will give her a chance to get to know her cycles and find out if there are any issues, or if she just hasn't had the egg meet the sperm yet.

If her charts show that she is ovulating, the nursing will have no effect on her ability to concieve.
