Carol, in response to your post about the slow gaining infant with Reflux and
possibly FTT, I did my senior year thesis in Nursing School on FTT. I went to
school in Boston and at the time I was working with UMASS which had a very
progressive, multi member team approach. As you mentioned, it is very
important to observe the family in their own home to assess for interaction,
methods of feeding, parenting and child style and wether or not they are
congruent, etc. FTT is not a diagnosis in and of itself, it is a conditon, of
which the etiology is usually multifactorial. Most MD's are getting away from
the label of FTT as a diagnosis. Many biological conditions need to be ruled
out, especially the thousands of metabolic aberrations that could be present.
IMHO, it is beyond a nurse or a general doctor's scope to rule out metabolic
aberrations, the baby should really see a pediatric specialist. Abeta and
hypolipoproteinemia are two conditions that I vaguely remember but can't find
in my pathophysiology text. I think they have something to do with an
abscence of carrier proteins for cholesterol. Choleseterol, has important
cell strucuture and steroid production functions. In my pediatric text those
two conditions are listed as causes of diarrhea, profuse, steatorrhea, fat in
the stool. It does sound like there is a Knowledge deficit that is preventing
the mom from implementing your suggestions versus a blatant non-compliance.
Maybe she does not understand the importance of your recommendations. She may
have fed her first child the same way she is now and I am assuming he did
just fine, at least in her mind, hope that helps, Lisa Mooney RN BSN Where
