I agree that I have seen the Lactina pumps and cases come back disgusting!!!  We use cavicide and an assortment of tools equal to your list (including Q-tips and cake decorating tips cleaning tool).  Ask Kay Hoover about her study on how different rental stations clean their pumps, what the manufactures recommend, and what has shown up in pumps (live stuff!).  I don't know when or how she is going to present this information, but I can't wait!
