Thanks to all who responded with advice about possible causes of the
diarrhea. Suggestions I received were to increase feeding time on one breast
over 60-90 minute period, cystic fibrosis, and rotavirus.  I talked with mom
about increasing the use of one breast at time to increase the baby's access
to hind milk (baby feeds very quickly on each breast, but when mom pumps,
she get 3-4 ounces out of the breast in just 5 minutes).  Mom is still only
using breastmilk.
Here's something mom told me after I shared your advice with her...she had
caught her daughter chewing on newspaper a couple weeks ago, stopped her and
didn't think anything of it...last stool had some undigested newspaper in
it!  Could be the source of her problems with the slow wt gain just being
this child's norm (she is gaining and growing and both parents are short).
Mom and her pediatrician are watching closely and had already discussed
rotavirus and CF.
