The smallest, least obtrusive frenulum tissue has caused some of the worst
problems I have seen.  As LCs, we well understand that many different factors
enter into breastfeeding difficulties.  Many years ago I saw a mom that was
having a multitude of problems...on inspection the baby had a slight, if any
problem  (in my mind) with his frenulum.  The tongue extended past the gumline and
the tissue stretched well, it was thin and just didn't appear to be "the
problem."
After about three days of diligent attempts at correcting everything else, I
discussed the possiblity of having the frenulum cut with the mother. I had
arranged our favorite pediatrician to come in and cut it at the bedside if she
agreed. In a matter of two minutes it was over without bleeding and the baby
went to the breast beautifully for the first time.  It was amazing and a
testament to how changing one thing can make a world of difference, whatever we may
think!  The mother reported total comfort with an excellent latch that felt
entirely different. I have since seen many similar situations and I am grateful
for the reasonable pediatricians willing to accept our expertise and requests.
