Having a short mandible (lower jaw) can make it more difficult for the 
baby to suck properly. Sometimes getting mom and baby sidelying in bed 
will help bring baby's head back enough that the lower jaw can get into 
the breast well. Encourage mom to have baby's body lower than she thinks 
she should, so he has to reach up for the nipple, and tip his head way 
back. She can pull him close to her by the small of his back, this tips 
his head even more.
It's a myth that babies cannot swallow with their head extended. Their 
soft palate and epiglottis touch, and their larynx is much higher than 
adults.
