I am an IBCLC and a BA, not a RN and in private practice, and I have a
provider number.
    To get a provider number is amazingly easy.  First step is to get a
federal ID number, usually your bank can get that for you over the phone or
call or go to the IRS, it takes just minutes to fill out the form.  Second
step is to call your state tax & revenue department and get a tax ID number
for state gross receipts tax, of course, then you have to plan to file tax
forms, even if you have no income.  Third step is to get a city business
licensee,
again a easy thing to do.  You can do the 1st three things in about an hour
and generally in that order as you may be asked for the previous number.
You need all of the above to get the provider number.   Then you go back to
the state and request a form for the provider number.  When you have the 1st
three numbers, fill out the provider number form as much as you can, then
call the state and ask questions about the few empty spots left on the form.
Mail it in with proof of malpractice insurance and a copy of your IBCLC
certification.  You may have to hunt for the right phone numbers, but it's
not too hard to find them.
