well--I am sure there is quite a lot in the archives already...but to 
reiterate:

If the baby has an allergy to cowsmilk, it is not the lactose he is allergic 
to, it is the protein.  I assume she is avoiding all cowsmilk products? 
Reading labels?  Another possibility, though, would be that mom has way too 
much milk, which would account for the excessive gain and the spitting 
up...and even the blood, I suppose, if it had gone on long enough.

How much can mom pump?  Has anyone weighed the baby before and after 
feeding?  If she has too much milk, and is feeding both sides at a feeding, 
she needs to down-regulate her supply by spending 3 hours per side to 
decrease supply to manageable levels.

Does the baby have eczema on his face?  Nasal congestion?  Infrequent 
stools?  Family history of allergy, including hay fever/asthma?

If the baby has had this extensive a work-up and nothing serious has shown 
up, AND he is thriving and happy, I would not do anything.  This does not 
sound like a metabolic disorder.
